The Admissions Process

We understand that the transition into a long-term care home is a change in your lifestyle, whether coming from a hospital or the community, and we will strive to help make it as smooth as possible.
In Ontario, the local Community Care Access Centre (CCAC) coordinates all applications to long-term care homes.
Here are the three main steps in the process:
- CCAC will assess your needs to ensure long-term care is the right care setting for you.
- CCAC provides you with lists, including wait times, and other basic information about the homes in your community.
- CCAC can help you choose and apply to a maximum of five homes.
